The Yellow Bracelet is a project aimed at people who have a domiciliary package of care
The Yellow Bracelet allows medical professionals to view a person’s domiciliary care package details in real time regardless of care setting.
Each person with a domiciliary care package will have a Yellow Bracelet with a unique QR code containing their care package information. The aim of the project is to:
-Reduce avoidable admission
-Reduce delays to discharge for patients in hospital who have a care package in place
-Reduced bed days spent in hospital
-Reduce avoidable conveyance
-Provide better outcomes for people
-Allow domiciliary care providers to maintain active control over their care packages
